1. SMS Program

These SMS Terms & Conditions govern the text-messaging program operated by ECB Humacao, LLC, doing business as Eye Center Boutique ("Eye Center Boutique," "ECB," "we," "us," or "our").

Eye Center Boutique offers Customer Care and Account Notification SMS messages. Message frequency varies. Message & data rates may apply. Text HELP for help. Text STOP to unsubscribe.

2. Types of Messages

After providing consent, you may receive Customer Care or Account Notification messages, such as:

• Appointment confirmations;
• Appointment reminders;
• Appointment rescheduling or cancellation notices;
• Responses to questions or service requests;
• Follow-up messages related to an inquiry you submitted;
• Notices regarding your account or existing customer relationship;
• Customer-service communications requested or initiated by you;
• Operational notices that affect an appointment or requested service.

The SMS program is not intended to send advertising or promotional offers.

3. Consent to Receive SMS Messages

By voluntarily providing your mobile telephone number and agreeing to receive text messages, you consent to receive recurring automated or manually sent Customer Care and Account Notification SMS messages from Eye Center Boutique.

Your consent applies only to the mobile number you provide.

Consent to receive SMS messages is not a condition of purchasing goods or services from Eye Center Boutique.

Message frequency varies.

The number of messages you receive will depend on your appointments, account activity, customer-service requests, and interactions with Eye Center Boutique.

Message & data rates may apply.

Eye Center Boutique does not charge a separate fee for participating in the SMS program. Your wireless carrier may charge message, data, roaming, or other fees under your mobile-service plan.

Contact your wireless carrier for information about your plan and applicable charges.

6. Opting Out

You may unsubscribe from Eye Center Boutique SMS messages at any time by replying:

STOP

After submitting STOP, you may receive one final confirmation message stating that you have been unsubscribed. Following that confirmation, you will no longer receive messages from the applicable SMS program unless you opt in again.

Opting out of SMS messages does not prevent Eye Center Boutique from contacting you through other permitted methods when necessary.

7. Getting Help

• Reply HELP to any Eye Center Boutique SMS message; or

8. Telephone Number Accuracy

By providing a telephone number, you represent that:

• The number is accurate and current;
• You are the subscriber, customary user, or authorized user of the number;
• You are authorized to provide consent for that number.

You agree to notify Eye Center Boutique if your telephone number changes or is reassigned.

Eye Center Boutique is not responsible for messages delivered to another person because you failed to notify us that your telephone number changed or was reassigned.

9. Privacy

Information collected through the SMS program is handled in accordance with the Eye Center Boutique Privacy Policy.

We do not sell or share mobile information or SMS consent records with third parties or affiliates for their own marketing or promotional purposes.

Vendors supporting message transmission may process limited information only as necessary to deliver and operate the SMS service.

10. Carrier and Delivery Disclaimer

SMS delivery is subject to wireless-carrier networks, device compatibility, network availability, and other factors outside Eye Center Boutique's control.

Messages may be delayed, blocked, duplicated, or undelivered. Eye Center Boutique and participating carriers are not responsible for delayed or undelivered messages.

Not all mobile devices or carriers may support all SMS features.

11. Security and Medical Emergencies

SMS is not guaranteed to be secure or confidential. Do not send detailed medical information, payment-card information, Social Security numbers, passwords, or other highly sensitive information by SMS.

The SMS program must not be used for medical emergencies. For a medical emergency, call 911 or seek immediate emergency assistance.

12. Program Changes or Termination

Eye Center Boutique may modify, suspend, or discontinue the SMS program or these Terms & Conditions at any time.

Changes will be posted on our website with an updated effective date. Continued participation in the SMS program after an update constitutes acceptance of the revised terms.
